The accumulation of yellow-brown, haemosiderin-laden, pigmented macrophages (“smokers’ macrophages”) within the lumens of respiratory bronchioles and into the peribronchiolar alveolar spaces is seen very frequently and in the great majority of cases secondary to a cigarette smoking habit [8–11]. Macrophages initially fill the lumen of the respiratory bronchioles and peribronchiolar alveoli leading to a specific type of bronchiolitis, namely respiratory bronchiolitis.

The normal iron content of hemosiderin can be detected as blue to purple particles using Perls Prussian blue stain. The presence of hemosiderin-laden macrophages indicates prior neural injury with associated hemorrhage resulting in macrophage phagocytic degradation of extravasated red blood cells and their hemoglobin content. 2017-09-12 · Iron-deposition is a metabolic biomarker of macrophages in both normal and pathological situations, but the presence of iron in tumor and metastasis-associated macrophages is not known. Here we mapped and quantified hemosiderin-laden macrophage (HLM) deposits in murine models of metastatic breast cancer using iron and macrophage histology, and in vivo MRI. Iron MRI detected high-iron pixel Hemosiderin-laden macrophages could not be demonstrated at autopsy in patient 1 four weeks after an acute, diffuse bleeding episode. These observations suggest clearance of hemosiderin-laden macrophages from airways within two weeks, and total clearance of these macrophages from the lung within two to four weeks after an acute hemorrhage.

Hemosiderin-laden macrophages may be seen as a morphologic evidence of remote hemorrhage (Figure 1). Dystrophic calcification may occur in older lesions. The morphologic examination of fat necrosis is usually straightforward.

Figure 1 Iron is stored as hemosiderin in resident macrophages of the bone marrow. An increase in the amount of hemosiderin within the bone marrow is recorded as (increased) pigment. Hemosiderin appears as a golden brown pigment with hematoxylin and eosin stain and as green, blue-green, or black with Wright’s stain. Although golden brown pigment seen in macrophages most likely represents hemosiderin, a special stain, such as Prussian blue, would be needed for confirmation.

We hypothesize two reasons for this: (i) occurrence of an intratumoral hemorrhage after biopsy; or (ii) tumor hypervascularity with occurrence of spontaneous intratumoral hemorrhage.
